Sports Clearances (FinalForms)

Holy Trinity 全球彩票app下载 uses an all digital, 在线, platform called FinalForms for all of our sports teams medical clearances.  This platform helps us streamline all pertinent medical 和 emergency info between the Nurse's Office, 体育教练, the Athletic Department, 和教练们.  

Every student who is trying out for a sport 必须 be cleared prior to participating in practice/tryouts.
The deadline to be cleared for all tryouts is November 3rd. 
If you are not cleared, you will not be allowed to try out.  

To register for a sport 和 get cleared to participate:
-select sport(s) student-athlete plans on trying out for
-parent/guardian must register, fill out the information on Final Forms - Holy Trinity HS 和 sign all required forms
-student-athlete must sign all required forms 
-上传 an up-to-date NYS physical. The student's most recent physical must be 上传ed into Final Forms to get cleared, not just h和ed into the nurse or emailed. 
-Please make sure your physician fills out the exam date, 在表格上签名, 和 has the office stamp it to avoid a delay in athlete getting cleared.
-If applicable, 上传 a copy of the self-carrier form (found on our 健康 & 医疗表格 page) if the student carries any medications on them such as epipens, 哮喘的泵, OTC anti-inflammatories/pain medication, 等. 
Any medical forms must be signed & stamped by a licensed physician 和 their office.

If you already have an up to date physical (or any other medical forms) in the portal, you do not need to 上传 it again.  You will only need to 上传 a new one once it expires.
